The clinical aspects of HoFH are mainly based on small and regional, often observational, studies. The natural history of HoFH and the relevance of predictive biomarkers are largely unknown. Treatments vary widely by region. Despite existing diagnosis and treatment guidelines, large regional differences are present regarding diagnosis and accessibility of treatments.

Multinational collaboration is essential to increase understanding of HoFH. The HoFH International Clinical Collaboration (HICC) was established to create an international network of healthcare providers to share data and knowledge on HoFH patients.
